We need an app ?

You will only need the App the first time to associate the card with your profile, after which you can continue to use it to modify your contact or use only the web access.
How I create the profile and when I have to do it ?
You can create it before or after purchasing your card. When you receive it, you will need it to associate it with the card.
You can create it from the site or directly via the App.
